Description
Direct oral anticoagulants (DOACs), such as apixaban and rivaroxaban, arrived on the market with no need for monitoring. As the use of DOACs expand, it is becoming clearer that their measurement may be necessary in specific situations, and more institutions are integrating either qualitative or quantitative assays. To help you stay abreast of current medical thought, use cases and audience polling examples are included to illustrate and prepare for new testing. Additionally, this activity addresses questions such as: Which type of assay is most appropriate for your institution? How do you validate these assays, many of which are not FDA-cleared? When should they be used? How do you integrate the testing in the ordering menu for clinicians?.

This activity was originally presented at CAP23 as part of the activity Optimize or Implement Lab Testing for Xa-Inhibitory Direct Oral Anticoagulant (DOAC) Detection.

Objectives
After completing this activity, you should be able to:

  1. Describe indications for measurement of apixaban and rivaroxaban.
  2. Describe different types of assays for quantitative or qualitative assessment of these anticoagulants.
  3. Approach bringing the appropriate assay for your institution in-house, including validation and integration into the ordering system/electronic medical record.
