Rereleased Activity
This activity was originally released as 2017 CPIP-D Case 4: Immunology - Detecting Latent Mycobacterium tuberculosis Infection Using Interferon-Gamma Release Assays and was available between April 17, 2017 and April 16, 2020. The content was reviewed in March, 2020. If you completed previous versions, you are not eligible to claim CME/SAM credit(s) for completing this updated version.

Description
The tuberculin skin test (TST) has been a common test to screen individuals for latent Mycobacterium tuberculosis (TB). Recently developed interferon-gamma release assays (IGRAs) are generally more sensitive and specific than TST to aid in the diagnosis of latent TB. This activity reviews TB screening tests, provides guidance for selecting the appropriate screening test, and provides recommendations for follow-up testing.

This clinical pathology activity is intended for pathologists.

Objectives
After completing this SAM eligible activity and posttest, you should be able to:

  1. State the epidemiology of Mycobacterium tuberculosis (TB) infection and indications for screening, including identifying which individuals should be screened for TB.
  2. Discuss the advantages and disadvantages of interferon-gamma releasing assays (IGRAs) compared to the tuberculin skin test (TST).
  3. Describe how IGRA results should be interpreted and recommendations for follow-up testing.
  4. Distinguish tests for screening for TB exposure (latent TB) to those used for diagnosis of active TB infection.
