Description

This activity will focus on important aspects in diagnosing brain tumors in the era of molecular diagnosis, presented by a neurosurgeon introducing cases and a pathologist identifying crucial diagnostic aspects. They provide guidance on key elements of the final pathology report that are critical when making treatment decisions for patients with infiltrating gliomas. Also discussed are identifying features that aid in distinguishing infiltrating gliomas from other types of glial tumors and cortical dysplasia. You will learn to distinguish key features using an algorithmic approach to their identification and see real-life scenarios during a discussion incorporating the neuropathology, neurosurgery, and neuro-oncology perspectives.
This activity was originally presented at CAP24 as Brain Tumor Diagnosis in the Molecular Era: What the Neurosurgeon and Neuro-Oncologist Need to Know.

            Objectives

After completing this activity, you should be able to:

          - Identify key MRI features that are helpful to the pathologist in the preoperative differential diagnosis of infiltrating gliomas.
- Recognize key histologic findings for the diagnosis of glioblastoma IDH-wildtype; astrocytoma IDH-mutant; and oligodendroglioma-IDH-mutant and 1p/19q codeleted.
- Describe the important molecular alterations that influence diagnosis and grading of infiltrating gliomas and when molecular testing is required.
- Explain important aspects of evaluating and reporting cases in which surgery is performed to distinguish recurrent tumor (true progression) versus radiation necrosis (pseudoprogression).
